Occasionally it's genuinely the agent or the marketing. Far more often it's one of three things: the price was above what the condition justified, the property has something buyers won't take on, or the buyers who did want it couldn't get financing on it.
That last one catches people out. A house can attract genuine interest and still fail, repeatedly, because every offer comes from a buyer whose lender declines the property after the inspection or the appraisal. The seller experiences that as bad luck. It's usually structural to the house.
